  5. needed to complete the form, and in addition, the limitations

  12. Fill in the blank boxes-- Enter the information. You can enter
  13. over a single line worth of data. If you make an error before
  14. entering the information you can use the backspace key to erase 
  15. information.

  17. Multiple choice screens-- The choices are mutually exclusive, pick
  18. the one which is applicable.

  20. Yes/No screens-- Select yes or no.
